Community Link Worker

Our Community Link Workers are non-clinical professionals that offer one-to-one support that is confidential, person-centred, regular and in a way that works best for you. 

The Community Link Worker wants to get to know you better, so with your permission, will ask questions and listen to you about your interests, likes and dislikes and what is going on for you currently so that they can work with you to develop, plan, and help you to achieve your goals. They may then socially prescribe you a service that they think will support needs that you have identified.

We work with people who face multiple and complex challenges including homelessness and poor housing conditions, loneliness, mental health, physical health, language barriers, addictions, relationship breakdowns and more with an aim to improve their health and wellbeing. 

We can support patients with:

  • Being involved in the community
  • Employment, education, or volunteering
  • Finances including household income and budgeting
  • Lifestyle Changes – for example attending a walking group

We have three Community Link Workers who can offer between 4-6 sessions either face-to face, via phone, or online appointments.  We can also meet you in the community and accompany you on visits.

  • You give us permission to
  • We are concerned about your safety, or we are concerned about the safety of someone else.
